〈SIRSの出現の背景〉

 Marshall1)は、この約20年で得られた生体反応の理解、すなわち、感染症の重症度は、微生物またはそれに由来する毒性物質によるよりは宿主側の反応に依存するとする理解は、19世紀に始まった病原微生物の発見とこれに続く抗生物資の発見に匹敵する程の進歩であろうと述べている。
 全身性炎症反応症候群(SIRS:systemic inflammatory response syndrome)は、感染症研究の過程から提唱された病態であるが、その誕生の歴史をみると、このような病態理解に基づいた治療法を臨床評価する際の患者のentry criteriaとして登場している。SIRS患者においてその効果が検証された治療法は、抗サイトカイン抗体の投与など、主に炎症のメディエーターを標的とした“mediator‐targeted therapy(以下MTTと略)”である。抗生物質が肺炎を治癒に導くといった、分かり易い関係とは趣を異にし、MTTの有効性の評価に関しては、その手段(指標)や評価基準の選択にも難渋する現状がある。

〈SIRSの定義〉

 SIRSは、1991年に開催されたアメリカ胸部疾患学会とアメリカCritical Care Medicine学会との合同カンファレンスの統一見解として提唱された病態概念であり、議長のR.C.Boneを筆頭著者としてそれぞれの機関誌「Chest」と「Critical Care Medicine」誌上に発表された2)3) 。これらによれば、以下のうち2項目以上を満たす場合に、SIRSと定義される。すなわち、 1)体温:<36℃または、>38℃、
2)脈拍数:90/分以上、
3)呼吸数:20回/分以上、またはPaCO2<32torrによって示される過換気の存在、
4)WBC:12,000/mm以上か4,000/mm3以下、または10%以上の幼若白血球(桿状核球)である。
このようにSIRSは、簡単な生理学的指標を用いて定義されたが、当時すでにSIRSにおける様々なメディエーターの役割が注目されており、その数年後にはR.C.Bone自身が、SIRSの背景をなすサイトカインなどのメディエーターの変動とその解釈、それらを解釈する上での問題点についての優れた総説4)をまとめている。

〈SIRSへの批判〉

 SIRSが発表された後、これに対しさまざまな異論が述べられた。Vincent JL5) は、このクライテリアでは、(1)感受性(sensitivity)が高過ぎ、ICU患者の2/3がSIRSに該当し、また特異性(specificity)にも欠け、患者がSIRSであると呼ぶことは“critically ill”患者であるというのに近く、患者群が不均一となり、炎症反応のモデュレーション治療などの有効性が隠蔽されてしまう、(2)SIRSは、病態生理の理解に役立たない、(3)病態の変化を反映しない等の問題点を挙げ、最後に、(4)SIRSには意味がなく、役立たないばかりか、しばしば背後に潜む感染源を探ろうという意欲を削ぐ結果にt繋がることが懸念されると結論づけている。
 これに対し、Dellinger RP、Bone RC6) は、本来SIRSは、臨床的に感染病巣が明らかでない場合でも、全身性の炎症反応を呈する患者をもれなく包含する目的を達成するものであり、この観点では、SIRSのクライテリアは、全身性炎症反応の最低限レベル(閾値レベル)を捉えており妥当であると反論している。また、本来、感染、熱傷、膵炎、外傷といった患者の臨床像を全く考慮せずにSIRSの判定を行うべきものではなく、一方、判定では簡単かつ客観的な生理学的指標を用いるため、多施設研究に適している点もあらためて指摘している。そもそもACCP/SCCM合同カンファレンスの論文2)3)にもその有効性を示すデータのほか、その限界も併記されており、改善の余地を残すものであることも明言されている。さらに抗エンドトキシン抗体の有効性を検証するプロジェクトではSIRSの基準のほかブドウ球菌感染症を除外項目として加えるなど、工夫の具体例も示されている。
 最近も依然として、SIRSやMODS(関連用語の項参照)に対する批判は続いている。Marshall JC 1) は、SIRSのentry criteriaによってMTTによる利益が得られる集団が捉えられているか否かについては不明であるとし、また、SIRS、MODSはそれぞれ一つの概念として取り扱うべきで、病態解明の研究には有用であろうが、臨床家にはSIRSを治療するのではなく疾患を治療するという認識が重要であると述べている7)。Baue AE8) もSIRS、MODSは単に人の気を引く頭字語(acronym)にすぎないと手厳しい。
 一方では、SIRSスコア2以上の外傷患者では、感染症併発のリスクが有意に高いとするもの9) など、SIRSの有効性も引き続き報告されている。

〈メディエーターとくにサイトカインを標的とした治療(mediator‐targeted therapy:MTT〉

 過去10年以上にわたり、抗エンドトキシン抗体、抗TNFモノクローナル抗体、IL−1受容体拮抗物質(IL−1ra)などによるMTTの有効性が、SIRS患者を対象としたrandomized controlled trial(以下RCT)によって検証された。基礎的研究での明らかな効果にもかかわらず、RCTでは期待を裏切られる結果となった。その原因の概要 1) を述べると、第一にSIRSのentry criteriaで選抜された患者群が、当該治療法による利益を証明できる集団になっていない場合がある。これには、
a)選ばれた患者の中に効果が期待出来る患者が全く含まれない場合、
b)適当な患者は含まれるものの、それ以外の患者も包含され、その効果が隠蔽されてしまう場合が含まれる。
第二は、各種メディエーターの変動の把握とその解釈が非常に難しいことに起因する。例えば、炎症局所に於けるサイトカインの動態が捉えにくいこと、合目的的なレベルか、過剰反応か、など、メディエーターの濃度の解釈がむずかしいこと、後述するが、抗炎症性サイトカインとの相互関係はどうか、などである。従って、メディエーター変動の解釈に誤りを生ずる可能性すら否定できない。この場合は、たとえクライテリアで適当な患者だけが抽出されたとしても、その効果に期待することは出来ない。

 第三には、適当な患者群が抽出されていながら、メディエーターの反応が感染防御や組織の修復などの観点から合目的的なレベルに留まる患者、これが過剰なレベルにある患者等、いろいろな段階の患者が包含される結果、同じMTTによって、ある患者では利益を得ているもの、他の患者では合目的的な生体反応の抑制からむしろ有害な効果がもたらされ、全体としての効果が示されないことも考えられる。Bone RC4)らは、SIRSをサイトカインの産生が局所に留まっているstage1、高サイトカイン血症が生体防御的なレベルであるstage2、有害な過剰反応レベルに達したstage3に分けているが、SIRSのstageによってMTTの効果に違いが生ずることには、概念的にも理解できる。

 いずれにせよ、十分な基礎研究に裏づけされている限りにおいては、SIRS患者を対象としたMTTの臨床試験は、今後も継続されることは重要であり、病態理解をさらに進める上でも重要である。

〈SIRSとCARS〉

 Roger C Boneら10)は、先述のようにRCTによるMTTの効果が得られなかったことから、CARS(counter anti‐inflammatory response syndrome)の病態を提唱しつつ、この問題を概説した。

 ところで、CARSは、SIRSのような臨床患者に対するクライテリアではなく、病態を示す概念である。すなわち、侵襲によって様々な炎症性メディエーター(pro‐inflammatory mediator)が放出されると、直ちにIL−4、IL−10、IL−1raなど、抗炎症性メディエーターの放出による制御機序が発動されるとし、これをCARSと呼んだ。両メディエーターの放出態度の違いによって、炎症反応をこれが局所に留まるものからMODSに至るまでの5段階(stage)に分けた。例えば、SIRSとCARSのバランスがとれ、炎症が終息に向かうものはstage2とされ、stage3からstage4に進むにつれCARSが次第に優勢となり、病態は免疫能の低下から臓器不全へと進行する。stage5は「immunologic dissonance」と表現され、過度の免疫抑制や炎症性メディエーターの著しい過剰状態であり、MODSもこの中に包含される。

 いずれにせよ、今後のSIRS研究とMTTの実施に当たっては、CARSの病態も併せて解明される必要がある。

 BACKGROUND:We have previously documented that the admission systemic inflammatory response syndrome(SIRS)score,calculated with four variabeles‐temperature,heart rete,neutrophil count,and respiratory rate−is a significant predictor of outcome in trauma(n=4,887).The objective of this current study was to validate our previous findings in a larger trauma patient population,to analyze the predictive accuracy of the four individual components of the SIRS score(temperature,heart rate,neutrophil count,and respiratory rate),and to assess whether the admission SIRS score is an accurate predictor of intensive care unit(ICU)resource use in trauma.METHODS:Prospective date were collected on 9,539 patients admitted to a Level I trauma center over a 30−month period(January 1997−July1999).Patients were stratified by age,sex,race,and Injury Severity Score(ISS).SIRS score was calculated at admission,and SIRS was defined as a SIRS score > or =2.

RESULTS:SIRS score was validated as a significant independent predictor of outcome in trauma by logistic regression analysis after controlling for age and ISS.Of the four SIRS variables,hypothermia(temperature < 36 degrees C)was the most significant predictor of mortality after controlling for age and ISS.

 Leukocytosis(neutrophil count > 12,000/mm3)was the most significant predictor of total hospital length of stay.SIRS scores of > or = 2 were increasingly predictive of mortality and ICU admission by logistic regression analysis(p<0.001).CONCLUSION:These date provide further validation that an admission SIRS score of > or = 2 is a significant independent predictor of outcome and ICU resource use in trauma.Temperature(hypothermia)is the individual component of the SIRS score with the greatest predictive accuracy.SIRS score should be calculated on all trauma admissions.

 OBJECTIVE:To determine the precise relationship between tissue factor and tissue factor pathway inhibitor(TFPI)after trauma,as well as to test the hypothesis that low TFPI levels are not sufficient to prevent tissue factor‐dependent intravascular coagulation,leading to multiple organ dysfunction syndrome(MODS).DESIGN:Prospective,observational cohort study.SETTING:Emergency room and intensive care unit in a university hospital.PATIENTS:Thirty‐three trauma patients,18 with disseminated intravascular coagulation(DIC)and 15 without DIC were studied.Ten normal,healthy volunteers served as control subjects.INTERVENTIONS:None.MEASUREMENTS AND MAIN RESULTS:Antigen concentration of tissue factor and TFPI,and global parameters of coagulation and fibrinolysis were measured on the day of admission,and on day 1−4 after admission.The number of systemic inflammatory response syndrome(SIRS)criteria that patients met and the DIC score were determined,simultaneously.The results of these measurements,incidence of MODS,and outcome were compared between the DIC patients and those without DIC.In the DIC patients,significantly higher tissue factor levels(p=.0049)and lower platelet counts(p=.0016)were found compared with the non‐DIC patients and control subjects.However,the TFPI values remained at normal levels during the study period.No correlation was found between the peak levels of tissue factor and TFPI.

 The mean duration of SIRS and the maximum number of the SIRS criteria being met by the patients in the DIC group were statistically longer and higher than those in the non‐DIC patients.The incidence of MODS and the number of the dysfunctioning organs were higher in the DIC patients compared with those in the non‐DIC patients,and the DIC patients had a poor outcome.CONCLUSIONS:We systematically elucidated the relationship between tissue factor and TFPI in post‐trauma patients.Highly activated tissue factor‐dependent coagulation pathway is not sufficiently prevented by the normal TFPI levels in patients with DIC.The DIC associated with thrombotic and inflammatory responses causes MODS,and leads to poor outcome in post‐trauma patients.

 Severe sepsis and probably most prolonged critical illnesses reflect a paradox of combined increased activation and depression of the immune apparatus.The increased activation of the inflammatory response is evidenced from the increased levels of circulating proinflammatory cytokines in the blood,increased endothelial activation with increased expression of inducible nitric oxide synthase,and increased de novo CD11b expression on circulating immune effector cells,such as PMNs,monocytes and lymphocytes.However,coexisting with this proinflammatory process is a profound anti‐inflammatory state characterized by increased circulating levels of anti‐inflammatory species that both directly block the binding of proinflammatory stimuli to their cell surface receptors(IL−1 ra,soluble TNF receptors)and also induce an anti‐inflammatory state on their own(IL−10,TFG‐beta).This humoral anti‐inflammatory state is mirrored at the cellular levels by decreased monocyte ability to process antigen,characterized by a reduced HLA‐DR expression and impaired PMN upregulation in response to clearly proinflammatory stimuli.Accordingly,severe sepsis reflects a combined pro‐ and anti‐inflammatory state.Both the pro‐ and anti‐inflammatory arms have protective and destructive aspects,making their modulation by treatment less predictable than if their actions were purely beneficial or detrimental.

サポートなしには恒常性を維持できない急性期重症患者の臓器機能障害をいう。合同カンファレンスでは“failure”は、在る、無し、に二分する用語であるのに比べ、“dysfunction”は連続的に機能障害を表現できる用語であるため、こちらが適当との見解である。ただし、acronymそのものについても種々の観点からその適否が論じられている。
